When you hold ctrl to click on the layer, make sure you are clicking on the rectangle with the T in it, not the layer name. If you click the layer name, nothing will happen. When you click the rectangle with the T in it, you get a selection of the text and that is what creates the mask for the smoke layer. Hope that helps someone else who finds that a little confusing.
